From the menu to the left you may download our full Application, which contains our client information and release forms, Ruff House terms and conditions, and Ruff House Policies. Please print a copy and complete the Client Application forms prior to your pet's first scheduled visit with us.

Our facilities are dedicated to the health, happiness and safety of each guest and our policies have been designed with this in mind. If you have any questions regarding our policies please do not hesitate to contact a client services specialist at 920.720.2772.

In addition to completing the above forms, a copy of your pet's current vaccination schedule is required. Please contact your veterinary clinic and have them fax this schedule to 920.720.2775.
